We need some information to grant you a MyChart account. After you submit this request, it may take up to a week for processing and verification. When you're approved, you will receive an email or a letter with your activation code and instructions on how to activate your MyChart account. Before we can schedule an appointment, we must first collect some patient information. Please contact your clinic with any questions.
No insurance? Our financial counselors will see if you qualify for discount programs.Still no coverage? Please note that we request a $60 deposit. You will then be billed for the remaining balance of services provided/rendered at the time of the appointment.
